Pre-Admission

Pre-admission is an important part of your hospital care. To ensure we can confirm your admission, financial and other arrangements, we ask that you:

Complete the admission form and medical history that you would have received from your doctor, as soon as possible and send it back to the hospital

Please have the following ready when you fill in these forms:

  • Personal/Next of Kin details

  • Medicare Card

  • Funding details (e.g. Let our staff know if you are using DVA, Private health insurance, or self-funding)

  • Any Benefit details (e.g. pharmacy benefit card or pension card)

  • The details of any medications that you take

  • Information your doctor supplied to you re implantable medical devices (eg prosthetic and disposables) – If applicable

If you have private health cover, please contact your health fund prior to your admission to check for any excess or waiting periods.

We will send you the Gold Coast Eye Hospital Informed Financial Consent. This document explains what your fees are and how to pay.

We know that health and billing charges can be difficult to understand and we are happy to assist in any way we can, however we also advise that you seek clarification from your doctor and health fund.

Nursing Pre-Admission

Nursing Pre-Admission

You will be contacted by telephone prior to your day of admission by our Pre-admission Nurse to discuss your care during your stay with us.

  • Your medical history will be reviewed with you and admission and discharge details discussed.

  • Tell us if you are allergic to anything

  • Tell us if you are diabetic

  • Tell us if you have any special dietary requirements you may have prior to your admission.

  • If you are having surgery with us, you are not allowed to eat or drink anything for 6 hours prior to your admission time. If you eat or drink within this time, your procedure may be cancelled or deferred.

  • Take this opportunity to discuss any concerns regarding your admission

  • You will need to have a carer who can pick you up from recovery, take you home and stay with you for 24hrs after the procedure. Our nurse will ask you who this person will be.

  • If you cannot organize a carer, your procedure will be deferred or cancelled.

  • Don’t  drink alcohol for 24 hours prior to surgery, or 24 hours following your procedure.

  • Your doctor will explain your procedure or operation and complete the consent form with you.
